چکیده
Background. A quasi-experimental retrospective study was undertaken to evaluate a new concept of free tissue volume restoration combined with bridging reconstruction plate (compartment approach) to reduce plate-related complication rates. Methods. We evaluated 40 patients with large lateral mandible defects and associated complex soft tissue defects reconstructed with a revascularized soft tissue flap and titanium hollow screw reconstruction plates. A case-control comparison was performed based on reconstruction type: restoration of soft tissue defect (conventional approach—group 1) versus over-reconstruction of soft tissue defect (compartment approach—group 2). Results. Plate exposure rate was 6 of 16 (38%) in group 1 versus 2 of 24 (8%) in group 2, and the difference was statistically significant (p 5 .04). The mean time to exposure was 10 months. Plate fracture rate was 6 of 23 (26.1%) in dentulous patients versus 1 of 17 (5.9%) in edentulous patients. Gastrostomy tube dependence was 6 of 16 (38%) in group 1 versus 6 of 24 (25%) in group 2. Conclusion. The ‘‘compartment approach’’ reduces plate exposure rate and gastrostomy tube dependence. Revascularized osseocutaneous reconstruction is still required in dentulous patients. متن کاملReconstruction of Lateral Mandibular Defects with Soft Tissue Loss: The Role of the Submental Flap
Introduction: Mandibular continuity defects after pathologic resections or traumatic events are difficult cases for reconstruction. Defects involving both hard and soft tissue loss are more challenging, because of problems in soft tissue coverage. The role of the submental flap in this regard is presented. Materials and Methods: In a retrospective study from the archived files of Ghaem Hospi...

Sometimes, because of lack of proper coverage of distal fractured legs, these fractures cause loss of the skin and expose the distal Tibial bones on the medial sides. In such situations, there are two ways for the soft tissue reconstruction: 1. Cross leg flaps and 2. using free flap with microvascular anastomosis. In the present article a case of free Gracilis flap transfer for reconstruction o...
متن کاملThe bridging lateral mandibular reconstruction plate revisited.
BACKGROUND Lateral oromandibular reconstruction using a soft tissue free flap with a first-generation locking mandibular reconstruction plate (MRP) was rejected in a previous series by the senior author (K.E.B.) owing to a high incidence of delayed plate extrusion through the cheek skin. OBJECTIVE To reexamine this method of reconstruction using a second-generation, low-profile MRP. PATIENT...
متن کاملSynchronous Soleus and Reverse Sural Flap for Large Soft Tissue Defect Reconstruction of Leg
BACKGROUND Extended Soft tissue defect of leg including middle and distal parts always has been a challenge for many plastic surgeons and also a frustrated problem for patients and families. To introduce the use of the soleus muscle and reverse sural flaps as synchronous surgical treatment alternative of the leg bone exposure with large soft tissue defect, this study was conducted. METHODS T...
